Wait, there is a rational egoism theory. If you don't take care of yourself you will become a burden to others so it is more rational to take care of yourself too because ironically if you don't you are selfish, because you will get sick or injured and others will have to take care of you. But no doubt people need to take care of other people too.

It is more about the psychological aspect of this circumstance. Caring about others before taking care of yourself - it distracts the mind well from your own problems.
Never seen that phenomenon when somebody takes care of others without an end of his energy, but his own life would also urgendly need a makeover?

Not only that.
In a subtle way, you also imply with your ego "somebody else is not able to make it alone".
You think of yourself being greater, able to do more than the object you focus on. You know... looking down on someone. The two parties are not equal.
